    Control panel
    Network connections
    Right click your connection and click properties.
    All the information you need should be there.

    I think you should call your provider to find out your modem address.

    what George said about your connection info…but the modem *address* ( I believe it’s called the MAC address) is on the modem itself. There’s a manufacturers tag on the modem that you have to supply your ISP. Mine is labled “manufacturers MAC address”
